We are excited to announce that Dr. Faraz will be taking over Dr. Amy Forbes’ practice starting May 1, 2024

From July 1, 2023 Dr Emily Allison will be the locum for Dr Coman-Wood. Dr Coman-Wood will be assisting with on call and follow up of previously ordered tests.

Non-urgent appointments can be booked via our online booking (the link is at the top of the page) or by emailing us at booking@portcreditmedical.ca

Please DO NOT call our office for Rx renewal. Instead, please ask your pharmacy to fax over the request several days before your medications are about to run out and expect up to 2 business days for completion.

Please DO NOT call our office to speak with our staff to review your test results. Any inquiries about test results require an appointment with your physician which can be booked via online or email booking. Certain results such as Mammogram via Ontario Breast Screening Program, FIT stool test for colorectal cancer screening, or PAP test for cervical cancer screening results will be mailed out to you by Cancer Care Ontario if they are normal.

COMPREHENSIVE PRIMARY CARE
As a primary care clinic, we focus on prevention and treatment of common medical problems and connecting patients with other levels of care including hospitals and specialist care. We also focus on health promotion, healthy child development and primary maternity care.
PREVENTIVE CARE
We strongly believe in the importance of preventive health care including screening of caner and preventable diseases, immunization, smoking cessation, bone health maintenance, and nutrition and lifestyle modification.
CHRONIC DISEASE MANAGEMENT
We provide proactive and comprehensive management for patients with chronic diseases, including high blood pressure, congestive heart failure, diabetes, etc. We focus on shifting chronic disease management from reactive, episodic care to a planned, proactive approach.
URGENT CARE
We provide urgent medical care to patients with non-life threatening acute illnesses such as common cold, flu, sore throat, earache, urinary tract infection, musculoskeletal injury, and minor cuts and burns.
